Role of Lymphadenectomy in Ovarian Cancer: Toward a Rational De-escalation Strategy

Abstract

The role of lymphadenectomy in ovarian cancer management has been significantly redefined in recent years. In advanced stages, the LION trial demonstrated no improvement in overall or progression-free survival with systematic lymphadenectomy in patients without clinically suspicious lymph nodes, while increasing surgical morbidity. These findings support a de-escalation of nodal surgery in this setting.
In early-stage disease, lymphadenectomy remains important for staging purposes; however, its systematic use is increasingly debated due to its limited diagnostic yield and associated complications.
Sentinel lymph node mapping is emerging as a promising alternative, allowing targeted nodal assessment while minimizing morbidity. Although evidence is still limited, this approach may reshape future surgical strategies.
Overall, current trends favor a more individualized approach, aiming to reduce treatment-related morbidity without compromising oncological outcomes.
