News on lower limb compression: equipment and method

Abstract

In 2025, news about compression therapy for the lower limbs mainly concerns the optimization of initial treatment, individualized pressure settings, technological innovations, and new recommendations on compression therapy for patients at risk of arterial problems, particularly those with mixed ulcers.
Currently, compression remains the standard treatment for chronic venous disease and venous leg ulcers.
An important development concerns primary care management: in cases of delayed vascular assessment, the immediate initiation of gentle compression (≤20 mmHg) is now proposed as a transitional strategy, in the absence of signs of critical ischemia, in order to avoid worsening venous hyperpressure and improve treatment adherence. Intensification of compression is recommended after confirmation of venous etiology. Compression devices are also evolving with innovative materials that aim to improve the consistency of the pressure applied, ergonomics, and compliance, but their use remains limited due to their cost and significant out-of-pocket expense. In addition, standardization of training in the application of compression devices is also encouraged to ensure therapeutic efficacy.
In summary, current trends favor a pragmatic, safe, and personalized approach, integrating technological innovations and adapting compression to the patient's tolerance and vascular profile.
