Office-based surgery : what level of environment for what level of funding ?

Abstract

In France, the provision of healthcare services is divided between medical practices on the one hand, and healthcare establishments on the other. The recent publication of several legal texts raises the question of office-based surgery, and therefore of an intermediate environment. Office-based surgery thus finds itself at the crossroads of four evolutions (legal, anesthetic, surgical and organizational), with the consequence of defining and financing the most optimal locations for the management of surgical procedures that do not necessarily require anesthetic personnel, i.e. 2.6 million procedures per year (32% of surgical procedures performed in healthcare establishments), not counting interventional medicine and endoscopies.
In addition, the issue of quality/safety of care is paramount, as no surgical procedure is harmless. It therefore appears necessary to regulate this practice, through regulations, accreditations and/or guidelines, and to define appropriate funding, as well as the ONDAM envelope impacted: city, hospital, new envelope, etc.
