Distal Tibia Osteotomies and Joint Preservation

Abstract

The indications for supramalleolar osteotomy are numerous. They include the correction of the deformity, the preservation of the joint and also as a step in a procedure for other operations, such as total ankle prosthesis or tibiotalocalcaneal arthrodesis.
The goal of the osteotomy is to realign the ankle. Opening osteotomy has the advantage of avoiding shortening, but increases the risk of nonunion or delayed union, and the need for a structural bone graft.
The closing wedge osteotomy is easier to perform but is responsible for shortening. Dome osteotomy is used in cases of complex deformity. In cases of malunion after ankle fracture, simultaneous arthroscopic debridement is desirable.
