Abstract

The neuropelveological approach is undeniably a specific practice requiring knowledge and a technique that must remain in expert hands. In this sense, it is an innovative therapeutic weapon whose indications must be well defined.
The approach of the pelvic and/or perineal nerves is particularly interesting for the treatment of overactive bladder, incontinence, and for the treatment by neuromodulation of chronic neuropathic pain in this region. However, it is important to name things correctly and to distinguish between what belongs to the perineum or the pelvis. In this sense the name of the target nervous elements must be precise and in accordance with the terminologia anatomica. Some conflicts are not accessible by laparoscopy. However, they should not be forgotten.
The extension of the technique in the fields of the (rare) traumatic pathology of the lumbosacral plexus, the restoration of locomotion for the spinal cord injured must be part of the general scheme of what already exists and take advantage of the knowledge acquired from competent teams in front of these tables. Thus, the neologism of parapleology should not exclude the competence of neurological MPR teams and constant monitoring of patients during multidisciplinary consultations. Neuropelveology finds an interesting place there within processes that have already proven their worth. A technique cannot be the panacea and cannot become a discipline. It is a therapeutic weapon that must be known and used within the processes offered elsewhere.
The extension against osteoporosis, muscular atrophy, certain cardiovascular pathologies, the skin protection of patients with reduced mobility and then the sarcopenia of age is commendable but it must not fall into the current spectrum of posthumanism which does not is a profit only for a wealthy population and poses a disturbing philosophical problem: the exclusive control of the homo on its future. The augmented man is an elitist ideology that risks increasing already unbearable inequalities.
