Functional tricuspid insufficiency is associated with a significant operative mortality. Two percutaneous solutions appear promising.

Abstract

Edge-to-edge repair consists in anchoring the anterior leaflet to the septal leaflet using clips. The Transluminate study shows a procedure success rate of >80%, a low peri-procedural mortality rate (<1%) but a residual leakage rate of 50%. Clip-treated patients had an improvement in quality of life but no improvement in one-year survival.

Several percutaneous valves are under clinical validation. They offer the same safety in terms of the procedure risk (mortality <1%) but with the advantage of not leaving any residual leak. Clinical studies are ongoing to assess the clinical benefit.

Conclusion: Percutaneous solutions reduce the risk associated with the correction of tricuspid leaks but additional studies are needed to define their position in the therapeutic strategy.
