Focus on penile surgery

Abstract

Indications for penile surgery have been clarified and a focus at the French Academy of Surgery has been made.
For Peyronie‘s disease, surgery remains the gold-standard despite the emerging trend of intralesional injection of clostridium histolyticum. For congenital penile curvature, less frequent, the surgery consists in doing a corporeoplasty with the Nesbit procedure for instance, but the optimal age to perform the surgery remains debated. The penile implant is the gold standard for the management of erectile dysfunction with unsuccessful pharmacological or mechanical approaches. Recent progress have increased outcomes with a low rate of complications. This later technique remains, however, rarely performed. Penoplasties are more and more discussed, a gradation of patients on the penile size, intercourse possibility and presence of dysmorphophobia is mandatory. Aesthetic surgical procedures have a low rate of satisfaction with a high rate of complications. In case of a true penile size anomaly, the phalloplasty (close to gender reassigning procedures) are of a great interest. Overall, all procedures on genitalia need a global assessment including a psychological evaluation.
