For a long time, surgery of the sinonasal cavities was performed using microscopic approaches, with a limited view of a narrow operating field, and by external approaches associated with a significant morbidity. Since the 1990s, endoscopically guided surgery has gradually made it possible to treat almost all sinonasal pathologies by transnasal approach. The objectives of this presentation are (i) to describe the principles of endonasal endoscopic surgery, (ii) to summarize the main validated indications and (iii) to discuss the limits and future stakes of the technique.
