Pediatric Liver Transplantation : Single Center’s Experience

Résumé

AbstractPediatric liver transplantation developed quickly in recent years in China. In our center, we have performed more than 200 cases pediatric liver transplantation. The overall survival rate is 90.5%, 89.5%, 89.5% for 1,2,3 years respectively. LDLT are 138 cases and 1,2,3 years survival is 93.6% respectively, DCD donor’s transplant are 45cases and 1,2years survival is 95.7% respectively, SLT are 27cases and 1,2,3 years survival is 73.7%,69.8%,69.8% respectively.In order to avoid large for size syndrome we choice reduced or hyper reduced left lateral lobe as graft according to GRWR and donor liver characteristic in LDLT. For more than one orifice in graft’s outflow reconstructions on back table can made the graft implant easier. When pediatric recipient’s portal vein is less than 5mm in diameter, vessel interposition or enlarged by vessel patch should be perform to decrease the possibility of PV thrombosis. The most severe complication of DCD donor’s transplant is hepatic artery thrombosis and morbidity is 13.5%.For metabolic liver disease , we attempt to find a new way to solve the metabolic problem according to auxiliary liver transplant from the donor who has another metabolic liver disease . We performed several cases successfully and hope to solve this problem by exchanging partial liver between patients who have different metabolic liver diseases.
