Traumatismes de la main : Que reste-t-il du traitement tout en un temps avec mobilisation précoce (TTMP) un demi-siècle plus tard ?

Abstract
Hand surgery is a surgical speciality inherited from traumatology surgery and plastic surgery.
Gradually, free flaps and pediculated flaps has been more and more studied to help protect tissues.
Therefore in 1976, J. MICHON, G. FOUCHER and M. MERLE created the TTMP concept (for One-Time Treatment with Early Mobilisation in French), to help repair all the damaged tissues, and start having an early remobilisation, providing better functional results. In consequence, hand (injuries) emergency centres have been created, names “SOS Mains” center, all over France, thanks to R. VILAIN.
Fifty years later, due to better prevention at work, and development of industrial machines, there are less work accidents. Still, more than 2 millions hand injuries occur each year in France.
Hand injury emergency care has been altered over the years, for economic reasons and surgery teaching difficulties. Because of a 6-hour needed time to repair vessels, to reduce edema and tissue necrosis, getting a microsurgery equipped OR is still a daily problem, because of these special OR costs. That’s why there is a slow decline of patients medical and surgical care.
It is now time to develop good medical care for hand injuries, since surgical techniques are really high level but are not used as they should be, due to human and financial obstacles, to provide microsurgical.
