Abstract

Patients are cared for by pediatric teams up to the age of 18, with a caregiver-patient relationship that includes their parents. Care for walking children with cerebral palsy is global, and aims to limit the onset of deformities associated with the pathology and growth, largely through the use of orthopaedic devices. In adulthood, empowerment and the development of a life project are at the heart of the patient-caregiver relationship. Orthopedic problems are more often focal, with occasional aesthetic requests.

Assessment tools (video analysis, physiotherapy assessment, GMFM, CGA) and support tools (therapeutic education days, peer emulation, HIIT, professional integration) are used routinely to make this transition as smooth as possible and respond to any problems encountered. Multilevels orthopedic surgery is an integral part of this personalized project: the detailed surgical program is drawn up during multidisciplinary consultation meetings.

In conclusion, the AJA transition is prepared for from childhood, is conceived as part of a multidisciplinary approach and is based on the continuity and coherence of the medical and individual project.
