Abstract

In the past, innovation in surgery was mainly carried out by one surgeon, behind the closed doors of the operating room, driven by inspiration or conviction. However, this type of practice is currently outdated as it is considered unethical and assessment is mandatory since the emergence of Evidence-Based medicine.

When the specificities of surgery (irreversibility, aggressiveness, variability) generate uncertain results, “first in humans” requires ethical reflection, as progress in surgery (and so, innovation) is necessary. In this presentation, we discuss ethical difficulties and issues related to scientific integrity in the early steps of surgical innovation.
