La nouvelle terminologie dans le domaine des échinococcoses - quand l’échinococcose kystique (EK) remplace l’hydatidose / New terminology in the field of echinococcoses: : “cystic echinococcosis (CE)” instead of “hydatidosis

Les échinococcoses et leur prise en charge impliquent l’intervention de nombreuses disciplines. L’histoire et les différentes langues des zones d’endémie ont multiplié les synonymes pour décrire ces maladies et les parasites qui en sont responsables. Ce foisonnement linguistique, avec parfois l’utilisation de termes scientifiquement inexacts, rend la communication entre professionnels difficile. Un important travail réalisé par l’Association Mondiale de l’Echinococcose, sur la base méthodologique du consensus formalisé, a permis une standardisation de la terminologie internationale en anglais, publiée par le journal ‘Parasite’. Un groupe de travail francophone multidisciplinaire, représentant les principales régions endémiques du monde francophone, a proposé une adaptation spécifique à la langue française, publiée dans le Bulletin de l’Académie Nationale de Médecine. Les changements adoptés portent sur : 1) la dénomination des différentes maladies associées au développement du métacestode des espèces d’échinocoques : « échinococcose kystique (EK)», «échinococcose alvéolaire », et « échinococcoses néotropicales » ; 2) la restriction de l’usage de l’adjectif « hydatique » au stade larvaire du cluster d’espèces Echinococcus granulosus sensu lato ; 3) l’harmonisation des expressions qui décrivent la structure des kystes, avec l’utilisation des expressions « couche germinale », « couche stratifiée », « couche adventitielle », et « kystes filles ». Enfin, le système de description des interventions chirurgicales dans l’EK, et son acronyme « AORC » identique en français et en anglais, ont été retenus. L’adoption de cette nouvelle terminologie est essentielle pour la cohérence des publications et des ouvrages pédagogiques et pour la comparaison des études cliniques chirurgicales.

Echinococcoses and their clinical management involve numerous disciplines. History and the different languages spoken in endemic areas have multiplied synonyms to describe these diseases and the parasites responsible for their occurrence. Such a linguistic profusion, which includes the use of scientifically erroneous terms, impairs the communication between professionals. The important work performed by the World Association of Echinococcosis, based on the ‘formalized consensus’ methodology, has allowed a standardization of the international terminology in English, published in the journal ‘Parasite. A multidisciplinary French-speaking working group, which represented all major French-speaking countries of the regions endemic for echinococcosis, proposed an adaptation specific to the French language, which was published in the Bulletin of the National Academy of Medicine. Adopted changes concern 1) the names of the various diseases associated with the development of Echinococcus spp. metacestode: “cystic echinococcosis/échinococcose kystique”, alveolar echinococcosis/échinococcose alvéolaire”, and “neotropical echinococcosis/échinococcoses néotropicales”; 2) the restriction of usage for the adjective “hydatid/hydatique” to the larval stage of the species which belong to the Echinococcus granulosus sensu lato cluster; 3) harmonization of the various expressions which describe cyst components: germinal layer/couche germinale”, “laminated layer/couche stratifiée”, “adventitial layer/couche adventitielle”, et “daughter cysts/kystes filles”. Finally, the system which describes surgical operations for cystic echinococcosis, and its acronym ‘AORC’, similar in English and in French, have been adopted. Global use of this new terminology is crucial for the coherence of scientific publication and educational textbooks, and for the comparison between surgical studies.
