Big false aneurysm complicating a Bentall procedure

Background:
The false aneurysm after a Bentall intervention is a rare complication observed in 1.6% of cases, it is a serious complication with a high morbidity and mortality after surgery.
Case report:
We report the case of a 60-year-old man operated in 2008 for a Bentall operation following a dissection with simple consequences. Lost view. He returns 14 years later for a beating cervical mass. CT angiography demonstrates a huge pseudoaneurysm of the ascending aorta, probably due to the release of the distal anastomosis. Operated in an emergency under deep hypothermia with circulatory arrest. Installation of a Dacron tube. The postoperative course was difficult, with renal insufficiency requiring dialysis and pneumopathy treated with antibiotics. Mis discharged on D10. Reviewed one month later with a good result on the control scanner.
Conclusion:
False aneurysm after cardiac surgery is a rare but very difficult event to manage. Surgery under circulatory arrest remains the only valid option.
