Abstract
Beside extreme emergency cases, they are many cases of mild emergencies where patient may be informed in spite of his hospitalization since he or she will be operated on one or two days later. Informed consent forms don’t fit because too long, too technical and written in an unappropriate way. A more concise form (one page recto only) is advocated to be used at the patient’s room or in the emergency room.
These forms could be used in planned surgery to inform patients in two steps. During the first step, the patient is given a regular information leaflet. At the next consultation, after having been able to read it, the concise form allows to make sure he has understood the first one and to keep a track of what has being said between the surgeon and the patient.
