Hybrid Salvage Treatment in Diabetic Foot

Abstract

Introduction: The diabetic foot is a serious and incapacitating complication of diabetes. Its management is complicated with a risk of major amputation and considerable mortality especially what occurs in patients who are often fragile and with multilevel arterial disease. Revascularization for limb salvage and particularly the heel strike is imperative.The hybrid treatment (combination conventional and endovascular surgery) can be an effective solution for these types of patients?Materials and methods: From January 2009 to January 2014, we performed a prospective, single-center study, based on analysis of a consecutive series of 125 patients with diabetes in chronic ischemia of the lower limbs treated with hybrid technique. Our patients have several risk factors and 16/125 patients (13%) underwent major amputation of the contra lateral limb.Results: Technical success was 97% (121 patients) with a major post-operative morbidity rate of 4,8% (6/125). On a mean follow up of 18.41 months and which involved 119 patients, the average rate of primary patency was 80.7% and secondary patency of 95.7%. Limb salvage at 30 days was 96.4% (115/119 patients), medium 83% (99/119 patients) and 63% late. Conclusion: The hybrid treatment in the management of multiple and multifocal arterial lesions represents a less invasive option effective and durable.
