Interest of the Mobile Unit of Cardiac Assistance (MUCA) in Critical Haemodynamic circumstances

Abstract

Thanks to the improvement of extracorporeal circulation devices(new oxygenators, percutaneous canulas), it is now possible tobring cardiac support to the patient’s bedside in the intensive careunit, or in the catheterism room. It allows stabilization of compromisedhaemodynamic conditions. The indications are myocarditis,poisonings with different medications, acute myocardial infarction,cardiac arrest. After haemodynamic stabilization, the patient can betransferred to the surgical unit for further management: classicalcardiac surgery, weaning of the circulatory support in case of myocardialrecovery, bridge to long term cardiac assistance with otherdevices, and heart transplantation. Since 2002, out of 257 patientstreated with mechanical circulatory assistance by the cardiac surgeryteam of La Pitié hospital, 70 patients were managed in differenthospitals in the Paris region with the help of the mobile unit MUCA.More than 40% of these very ill patients either were weaned ofthe support, or were treated by long-term cardiac assistance, orreceived a cardiac transplant.
