Abstract
Chronic cardiac failure is an increasing problem. Despite significantprogress in the understanding of the physiopathology and improvedmedical management, prognosis remains severe. Cardiactransplantation is now the gold standard in the management of thepatients who become unresponsive to optimal and tailored medicaltherapy. Organ shortage is an increasing problem which makesmandatory a strict patient selection.The spirit of Jean-Paul Cachera, one of the pioneers in thisadventure, is still alive in the definition of the optimal protocols andthe management of the transplantation program at Henri Mondor’sHospital.
