Bladder replacement after radical cystectomy for cancer : our experience

Abstract

Radical cystectomy is the gold standard regarding the treatment ofbladder cancer and bladder replacement, whenever possible it constitutesthe best choice concerning the treatment of urine drainage.Developed by M Camey and his team since 1958 at Foch Hospital,bladder replacement has evolved from the Camey I technique, using30 cm of tubularized ileum, to the Z technique using 45 cm of debutularizedileum which allows a spherical neo ileal bladder.From 1980 to 2000, 413 bladder replacements have been performedin the department of urology at Foch Hospital, allowing an evaluationof functional and oncological results and short and long termcomplications (day-continence > 90 % ; nocturnal continence > 71% ; total incontinence < 5 %). Moreover, these 20 years have permittedthe evolution of the technique toward a peri-operative simplificationwith constant improvement of the results.
